Sharing a bio-data template for Matrimony purposes. All fields may not be mandatory. Use it base on your requirement. You can get it printed once you fill required fields.
Alternatively, you can visit a print shop. They will have huge collection of bio-data templates from which you can choose your favorite. They will fill required details based on your inputs and give you printed copies.

Name(complete name including, your first name, middle name and Surname), Age:(completed years), Sex, Height, Weight, Complexion, Date-of-Birth, Place-of-Birth, Religion, Educational -Qualification, {Marital Status,Children,Hobbies,Habits}

Professional Info may include -
Job Status(working,not-working,unemployed,student), Job Profile, Job Description, Annual Income, Company Name

Family Info may include -

Father, Mother, Siblings and Grand Parents Names, their qualifications and Job Description, if applicable. Also you may include Family-type(nuclear,joint), Family- values(liberal,modern,traditional,orthodox), No.of family members.

Horoscope Info may include -

BirthStar, Gothram, Rasi

Contact info may include -

Your contact number or/and Contact numbers of elders who have time to talk to prospective matches ;). Also include TeluguMatrimony Id (some call it T-number. he hee)
